If not registered for Swim Team, must fill out Summer Lesson Registration form before first lesson purchase.

Please indicate Level 1-4 (approx. if unsure).

In the Personalization box please specify:

• Name and age of Swimmer
• Top 3 requested lesson times (30 minutes) between 9am-12:00pm

Group Lessons are for Levels 1-4 only.

• Levels may be combined or subject to cancellation, if we do not have enough participants.
• We will do our best to accommodate time requests.
